What is personal data?

What information do we collect from you?

We may collect personal information from you when you use our site. All information collected will be kept secure and will only be used for the purposes it was collected for, unless agreed otherwise. All information is collected in accordance to this privacy policy, Data Protection Laws, Clinical Records Retention periods and Clinical Confidentiality Guidelines.

Information that you provide to us:

  • Enquiries (via our website, email, telephone or in person) – We record your details in order to track and follow-up on enquiries, in addition to scheduling appointments. This may include but is not limited to information such as your name, email address or phone number. This allows us to supply you with the information that you have requested about our professional services.
  • On submitting an enquiry via our website, you may choose to subscribe to our email list. Our email subscribers receive the latest news from Dr. Strawford on treatments, services and offers. We use your name and email address to send you such updates.   • Patient Records – Information is collected as part of your registration with the Skin Excellence Clinics. This includes information such as your name, address, date of birth, email address, phone number, next of kin and medical history. This allows us to fulfil our obligations with you – for internal record keeping, for billing and accounting, to respond to queries, complaints or requests for further information. We may also use the information provided to help us improve our products and services. We gather such information because it is necessary to undertake services for you or is necessary for our legitimate interests in managing our business and improving our professional services, in addition to complying with our regulatory obligations. Failing to provide us with information we require may result in us being unable to provide treatment.
  • Leaving comments on our Site. When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection. An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
  • Media uploads – If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.
  • Contact forms and Cookies (i) If you leave a comment on our site you may opt-in to save your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year. (ii) If you have an account and you log in to this site, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser. (iii)When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year. If you select “Remember Me”, your login will persist for two weeks. If you log out of your account, the login cookies will be removed. (iv) If you edit or publish an article, an additional cookie will be saved in your browser. This cookie includes no personal data and simply indicates the post ID of the article you just edited. It expires after 1 day.
  • Embedded content from other websites – Articles on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited that other website.   • Website Analytics – who we share your data with and how long we retain your data. If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue. For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information. In addition, we use analytics to continue to improve the experience of visits to our website, within these analytics no personal data is viewed or stored – data from from any analytics software, is deleted by automated rule after 50 months.

We use online contact forms for on-site enquiries. If you make an enquiry, we’ll collect the following:

Sensitive information relating to your medical history will be kept confidential and only be disclosed to those involved with delivering your treatment.

* Other members of Skin Excellence Clinics staff as part of your ongoing care provisions, or for the purpose of analysing the business

* In order to provide some of our professional services, we may use the input of selected third parties, where this is necessary for the performance of our contract with you. This will require the disclosure to such third parties of your contact details, as well as further personal data about you with your consent which is relevant to the services they provide. Selected third parties such as laboratories for histology on moles etc and pharmacies for private prescriptions purposes. Opting out of sharing your information with these providers may affect our ability to treat you. All our suppliers have entered into appropriate confidentiality obligations and/or contractual data processing clauses with us.
